Next PPG meeting

The next PPG (Patient Participation Group) meeting will be here at the surgery on Wednesday 14th August at 6pm.  All patients are welcome, just turn up and find out a bit more about what goes on in the surgery.

Changes to Repeat Prescription Ordering from 30th June 2019

From 30th June pharmacies will no longer be able to order prescriptions on behalf of patients at this practice.  See our Latest News page for more information.

New Patient Registration

If you live within our practice area we are happy to accept you as a patient. Please see the lovely comments we've received from some of our current patients on our Results of Patient Survey page.

There is quite a bit of paperwork to fill in to become registered and so please pop in to collect a pack for each member of the household who wishes to register or download from the links below. Once you've completed the paperwork send it back to us (drop it in to reception, put it in our letterbox or scan and email) and we'll get you registered. It's very straightforward!

 

Anybody on regular medication will need to have a telephone consultation with one of our practice pharmacists before they can be prescribed more. 

 

Adult Registration pack

Please fill out all parts of the registration pack once you have downloaded it. We may not be able to register you if you haven't completed all relevant parts. Thanks very much.

You also need to download and complete the NHS GMS1 form for each person who wishes to register. Please download here. We can't register you without this form as this is an NHS requirement.

Child Registration pack (for children up to and including age 10)

Please fill out all parts of the registration pack once you have downloaded it. We may not be able to register your child if you haven't completed all relevant parts. Thanks very much.
